Total focus comes as a result of your philosopy of music/life. You must not care a bit what the audience thinks of your performance. You must not care if the audience is even there (while you are playing). You must not care a bit whether you are making mistakes or not. This is a philosophical position. When you are playing, you must care for nothing except the music itself, and the joy you have in playing it. It also is necessary then, that you must be totally convinced that the music you are playing is worthy of such focus.
